COVID-19 Consent to Vaccinate Form

Dear Saint Mary's International Families,
Saint Mary’s School is dedicated to the health and safety of our students. Therefore, we are encouraging all students to be vaccinated against COVID-19. This will safeguard students from serious disease as well as exempt them from quarantine in the event of exposure. Most importantly, vaccination is our best defense to keep our school open for in-person classes.
We recognize that some students may be coming from areas where the vaccines are not available. Therefore, with your permission, we are offering to facilitate the vaccination of any unimmunized international student.
Attached, please find an amended Consent to Treat Form with specific permission to vaccinate against COVID-19. If you would like for your student to participate in this opportunity, please sign the attached form and return it to nurses@sms.edu.

COVID-19 Consent to Vaccinate Form
Dear Saint Mary's International Famlies,
For the health and safety of our students, Saint Mary’s School (SMS) requires our international families to provide or purchase comprehensive US-based health insurance.
Your daughter was enrolled in the SMS-recommended student health insurance if your family has requested it or if your family did not provide proof of other eligible insurance. (If you have enrolled or provided proof of insurance, you may disregard this message.)
If you have already made alternate arrangements, please upload your insurance policy information to Magnus as part of the health insurance requirement. Once that is done, Sharon Holmes, Director of Binder Health Center, will remove your daughter from enrollment with no penalty to you.
The cost for 10 months is $1,995 and for 12 months is $2,225. If you requested 12 months last year, then you were renewed for 12 months this year. Otherwise, you have been signed up for 10 months. The cost of the insurance will be added to your school account. This election can be changed with your written request sent to nurses@sms.edu.
