Grandparent/Grandfriend Contact Information Form

Ensure your student's grandparents and/or grandfriend(s) are invited to Saint Mary's annual Grandparents and Grandfriends Day events and activities! Complete the form below and we will send a hard copy invitation and/or e-vite with an RSVP form. The annual deadline for submission is February 1. Entries added after this date will only receive an e-vite. (Please be sure we have a valid email address on file, if possible.) (Grandfriends may be an aunt, uncle, Godparent, or someone in your student's life equivalent to a grandparent.)
